any of y'all manage your emacs config with home-manager? do you put custom.el in there too?

not exactly. I have an integrated emacs set up, but it's provided by environment.systemPackages

adisbladis: the only sticking point i had was themes, emacs wants to go by what hashes are safe to load but those of course change occasionally when nixpkgs gets upgraded
